Employee Trips on Loose Power Cord, Fractures Ankle

At approximately 8:20 a.m. on August 12, 2007, Employee #1 was opening a metal roll-up window in the warehouse. He turned to walk away, and her right foot was caught in a power cord, causing her to trip and fall. Employee #1 fractured her right ankle and was hospitalized. The power cord, along with a loose phone cord was secured with zip ties following the accident to prevent reoccurrence.
