Investigation Summary
At approximately 12:30 p.m. on June 15, 2007, an employee was making a metal table for the process line and was cutting pieces of aluminum using the De Walt Model Number DW708, Serial Number 13834, a radial arm saw located in the maintenance department. As the employee was cutting the last piece, the part jammed and flew and hit the employee's thumb, severing the distal portion of his left thumb and shattering the tip of the bone. The employee was hospitalized and required surgery, which resulted in an amputation.
