Employee's Head and Back Injured in Fall from Ladder

At approximately 1:45 p.m. on September 27, 2007, Employee #1, a window washer, was washing the second floor exterior windows of a middle school campus building. He was using a folded 40 ft extension ladder with no tie off or second person supporting the ladder, despite having an elevating work platform next to him. When he descended the ladder it slipped and fell away leaving him clinging to a roof overhang. He lost his grip and fell approximately 15 ft to the asphalt pavement. He suffered an impact to his head and back.
