Investigation Summary
At approximately 11:15 a.m. on December 4, 2006, Employee #1 was a regular full-time employee working as a helper for Labor Ready Southwest, Inc. The employer is a temporary employment agency in Long Beach, CA, that supplies workers to the surrounding area. Employee #1 and a coworker were working at a site in Signal Hill, CA, assembling a warehouse storage rack system. Employee #1 leaned a metal beam up against a wire fence inside the company site. The beam slid and fell, striking Employee #1 on the back of his head and lacerating it. He was immediately taken by paramedics to the Long Beach Memorial Hospital for treatment. He was hospitalized.
