Investigation Summary
At approximately 1:00 p.m. on August 10, 2006, Employee #1, an A/C technician for a refrigeration company, was on the roof of a building, servicing a Rudd A/C unit, Model Number UPNO036C, Serial Number 4512F49894805. The employer specializes in the business of servicing, repairing and replacing industrial type A/C units for various businesses. As he was working, he fell through a skylight that was about 17 feet above the base of the building. Employee #1 was taken to Harbor UCLA Medical Center, where he died due to complications from head and other injuries sustained.
