Employee Is Killed by Falling Equipment

Employee #1 was assisting a forklift operator to move an approximately 3,200-lb pin roll stand from a production line. The equipment was located in an aisle that was too narrow to allow the forklift to directly approach the pin roll stand. When the forklift operator lifted the pin roll stand, one of the four equipment feet was unsupported by the concrete pad. The pin roll stand fell from the forks and crushed Employee #1's leg against the pad. Employee #1 was admitted to Norfolk General Hospital and died two days later.
