Employee's Leg and Foot Are Injured in Forklift Collision

On June 2, 2006, Employee #1 was backing a forklift in a warehouse, while speaking with another forklift operator, and with his left leg protruding beyond side of the forklift. Employee #1 collided with another forklift, causing crushing injuries to his left leg and foot.
