Investigation Summary
Employee #1 sustained a fractured right arm and other extensive bodily injuries from falling off a roof approximately 18-ft 2-in. to grade. Employee #1 was wearing a full fall arrest harness system; however, the lanyard extension he was using had an extra 20 ft added on to it so that his fall from the roof was not arrested. Employee #1 was hospitalized.
