Investigation Summary
April 28, 2006, Employee #1 was employed by Lee County Tree Experts, Inc., and was working from an aerial lift bucket truck to trim tree limbs, using a hydraulic pruner. He came in contact with the uninsulated phase (conductor) and neutral electrical lines causing a severe electrical arc flash across his chin and neck area. The electrical arc caused the employee's clothing and flesh to burn. Employee #1 fell unconscious in the elevated bucket, which was promptly lowered by his coworkers. Water was poured onto the employee to extinguish the fire from his body, and he was removed from the bucket by his coworkers. Repeated attempts of CPR by the employee's coworkers failed to resuscitate him. The work was being performed by Lee County Tree Experts, Inc. as a result of a contract with the Powell Valley Electrical Cooperative to clear trees/brush from their electrical transmission lines. It should be noted that the employer's former name was Creechs Tree Service for the purpose of VOSH history. The incident occurred near the extreme southwestern tip of the State of Virginia in the Rose Hill area of Lee County, on private property located off of State Route 670.
