Investigation Summary

At approximately 2:43 p.m. on June 13, 2007, Employee #1, a truck driver, was operating a 2007, Volvo 630 tractor trailer, Vehicle Identification Number 4V4NC9TH77N448093, pulling a 53 ft, 1996 Trailmobile semi-trailer, Vehicle Identification Number 1PT04KAR5T6008867. He dropped the empty trailer at a loading dock bay and parked the tractor trailer nearby. He waited while the trailer was loaded. At approximately 5:34 p.m. he was notified that the loading of the trailer was complete. He reattached the trailer and moved it to a nearby staging area, next to another tractor trailer, with the trailer doors secured open. He was following the established procedure at the site which was to move loaded trailers away from the dock, with the doors secured open, to the staging area, where the driver would close and secure the trailer doors. He exited the vehicle and proceeded to the back of the truck without engaging the parking brakes. As he was closing the trailer doors, the trailer began to roll. He ran towards the front of the truck and attempted to unlock the driver door but fell forward. He was caught between the tires of his truck and the tires of the adjacent truck and was killed.
