Investigation Summary
On April 24, 2006, Employee #1 and a coworker were involved in a siding operation. They set two extension ladders on a single stage mason-type tubular welded frame scaffold to use in the siding operation. Due to the pressure from the ladders, the scaffold rolled, causing both employees to fall 12 to 15 feet. Employee #1 suffered a broken wrist. He was hospitalized for treatment.
