Investigation Summary
At approximately 6:45 p.m. on April 26, 2006, Employee #1 was scarfing out and rewelding a steel frame work approximately 40 ft long by 3 ft wide by 14-in. high, weighing approximately 3,000 pounds. He placed the frame on two saw horses with an overhead crane and then clamped the edge, the14-in.side, with two C-clamps to the saw horses. He removed the crane, came back to the frame and attempted retightened the C-clamps. The frame fell, trapping his legs, fracturing both of them. He was hospitalized and had a heart attack before surgery and is now in a drug induced comma.
