Investigation Summary
At approximately 8:25 a.m. on February 27, 2006, Employee #1, a 65-year-old male, was found dead due to a heart attack that was not work related. At approximately 9:00 a.m. on February 27, 2006, Employee #2, a 32-year-old male suffered a non-work related anxiety attack and was treated and released from the hospital. At approximately 11:10 a.m. on February 27, 2006, Employee #3, a 61-year-old male complained of chest pain and was transported to the hospital. Employee #1 was admitted to the hospital. The chest pains were determined to be not work related.
