Employee Is Killed When Pinned by Forks of Forklift

On October 23, 2006, Employee #1 and a coworker were spreading the forks on a forklift. The semi-trailer they were preparing to unload was being moved forward as they had requested. The position of the trailer in relation to the tractor caused the back of the trailer to swing toward the forklift pinning Employee #1 in between. Employee #1 was killed.
