Investigation Summary
On August 23, 2006, Employee #1, a temporary hire of the City of Tulsa, OK, was assigned to work in the solid waste section on his first day of work. He was working from the back of a garbage truck. When the truck turned, the employee fell and hit his head on the pavement. An Emergency Medical Service Authority (EMSA) unit transported the employee to a hospital, where he was pronounced dead.
