Employee Is Killed When Struck by Concrete Masonry Unit Wall

On December 6, 2006, Employee #1 was stacking scaffold frames 35 ft to 40 ft south of the wall of his foreman. The foreman was removing debris with a skid steer loader from the partially demolished wall. The foreman passed between the wall and the scaffold stacking operation with the skid steer and then went to the debris pile on the opposite side of the wall. The employee unexpectedly began manually cleaning a small amount of demolition debris approximately six feet from the base of the 9 ft tall, concrete masonry unit (CMU) wall. The skid-steer loader approached the debris pile with the bucket lowered to the floor and began to push load the bucket with debris. This action created pressure against the wall, and it collapsed away from the skid steer and onto the employee. Crew members removed portions of the wall from the injured employee and notified EMS by calling 911. Employee #1 died.
