Investigation Summary
On November 15, 2006, Employee #1 and a coworker were closing up the end of a pipe inside located inside a trench. The coworker was inside the trench handing tools to Employee #1, who was outside the trench. As Employee #1 was reaching to grab a tool from his coworker, he lost his balance and fell into the trench. He struck his head on a pipe, causing a severe concussion and Killing Employee #1.
