Employee Is Struck by Forklift and Later Dies

At approximately 8:13 a.m. on March 28, 2011, Employee #1, a spotter, was standing on the dock behind Crane Number 1. He was preparing for the arrival of the second hatch lid from an afloat vessel. A coworker, who was operating a forklift, used the forklift to stack two buckets of container pins onto the tongues of a Caterpillar 90 MDL Number DPL40 powered industrial fork truck. The coworker then drove in the direction towards Employee #1, striking him with the forklift. Employee #1 sustained traumatic injuries that killed him. Emergency medical personnel were summoned. They arrived and transported to him a hospital, where he was pronounced dead.
