Employee killed in fall from scaffold platform

On August 4, 2007, Employee #1 was on the west side of the 33 ft high inside bracket work platform of a 39 ft high by 5 ft wide by 70 ft long fabricated frame walk-through type scaffold. He stepped across to the inside bracket work platform on the south side of the scaffold, lost his balance, and fell 33 ft to the plywood-covered concrete sidewalk. Employee #1 was killed. There were no guardrails on the scaffold and Employee #1 was not using any type of fall protection.
