Investigation Summary
On May 31, 2009, Employee #1 was checking a motion sensor on a sliding door between a freezer and a cooler. He was last observed standing off to the side of the door at the electrical panel for door. When found, he was lying on the dry concrete floor of the aisle in the door, with the door standing open. This was a soft canvas door that would not cause harm. Employee #1 sustained a fractured skull. He was transported to the hospital, where he died 9 days later.
