Investigation Summary
On March 29, 2006, Employee #1 was installing a sprinkler system for fire protection. He was climbing up on a stationary rack on his way to install the sprinkler system on the twelfth level of a structure, approximately 60 feet above the ground. He was wearing fall protection, but he was not provided with anchorage points for tying off. He was using a 10-foot modified ladder to reach each walking and working surface. When he reached the first level, approximately 15 feet above the ground, he fell. He sustained a concussion, and he was killed.
