Investigation Summary
Employee #1 was working on a pallet on which the plastic wrap and bundle straps had been cut and laid back. Apparently, the plastic wrapping or the bundle strap had caught on the pallet of product behind open pallet. Pallet product was stacked two-high. The pallet behind the open pallet was pulled over onto Employee #1 as he tried to remove plastic wrap and bundle straps from open pallet. Employee #1 was found lying on his back with his left foot hung in the wooden pallet on which he had been standing. The second-level bundle of product toppled over, struck Employee #1 in the chest, and killed him.
