Investigation Summary
At approximately 1:00 p.m. on October 25, 2006, the employee was the passenger in a pickup truck traveling East on Highway 140. The pickup slid on ice into the path of a West bound tanker truck, colliding with it. The employee was ejected from the vehicle and was pronounced dead at the scene by responding Emergency Medical Services.
