Investigation Summary

Investigation Nr: 202450383
Event: 01/09/2006
Employee Is Struck and Killed by Loader

On January 9, 2006, an untrained employee was directed by his supervisor to drive a goat/loader from one grove to another grove. The employee was not a designated goat/loader operator, and the day of the accident was the first time that he had ever operated it. As the goat/loader crossed the intersection, it struck three employees who were waiting for the labor bus. As a result of the accident, two employees received various leg fractures, while Employee #1 was fatally injured. The original narrative gives no other information about the two injured employees.
