Investigation Summary
On December 16, 2005, Employee #1 was working for a residential building contractor. He was working from a ladder jack scaffold, removing vinyl siding from an existing home. He was on a 12 inch by 12 foot walkboard, when it fell 18.5 feet to the ground, taking him with it. He landed in a sunken patch of earth and sustained a broken tibia and a dislocated lower leg. He was hospitalized. He was not using fall protection, and the walkboard was not secured to the ladder jacks. The ladders were spaced approximately 4.5 feet apart, with an overhang of approximately 2 feet at each end. To get to the walkboard, Employee #1 had climbed a ladder jack ladder; there was no separate access ladder. The ladders were not at an incline of 4:1, and one extension ladder was missing a rope. Employee #1 was not trained in scaffold safety requirements pertaining to scaffold construction and working on scaffolds. Finally, the scaffold was not erected under the supervision of a competent person.
