Investigation Summary

Investigation Nr: 202422168
Event: 05/30/2006
Employee's Finger Is Amputated by Fan

At approximately 12:30 p.m. on May 30, 2006, Employee #1, a mobile equipment mechanic was troubleshooting an air conditioner problem on a CAT 7770 water truck. The air conditioner unit sat inside the cab just to the right of the driver's seat. After locking out the truck, the mechanic removed the molded plastic covering the air vents on top of the unit. Under the cover the unit is flat metal, with 2 squirrel cage fans recessed on the passenger side of the unit. The mechanic removed the lockout and started the truck to troubleshoot the problem. After deciding that the truck would have to go to the maintenance shop, the mechanic turned to put the cover back on. His right ring finger contacted the rear fan. Employee #1 was treated for a partial amputation of the finger tip. Employee #1 was not hospitalized.
