Employee Is Killed in Fall from Ladder on Scaffold Platform

On December 30, 2005, Employee #1 was working on a 6-ft steel straight ladder, which was standing on a scaffold platform. The scaffold platform was 6 ft above grade, which made the total height approximately 10-12 ft. As Employee #1 was nailing in the flange around a new window, the scaffold was pushed backwards, causing him and the ladder to fall between the platform and the house. Employee #1 was killed.
