Employee Is Killed by Collapsing Wall

At approximately 10:45 a.m. on November 28, 2005, Employee #1, a laborer, was tamping dirt around a newly constructed footer for a parking garage. A wall had been partially demolished during a separate and earlier phase of the project by another contractor who was to remove and level the top of a preexisting wall bringing it to ground level. Several months later, when the employer of the employee was setting footers for the new parking garage structure. The wall then weakened and collapsed on the employee. Employee #1 was killed. The investigation identified that at the time of modification of the wall, the contractor did not stabilize the wall by backfilling dirt in front of the wall as directed in the specifications. It was also identified that work activity around the wall included work with heavy equipment that exposed the wall to severe vibration.
