Investigation Summary
At approximately 10:30 a.m. on November 14, 2006, Employee #1, a foreman, was working on the roof of a building taking measurements for the installation of a roof drain and a roof overflow drain. The roof was about 30 feet from ground level and Employee #1 was in close proximity to an unguarded opening that measured 20 inches wide by 30 feet long. As he was taking measurements, Employee #1 slipped and fell through the unguarded opening to the ground and was killed.
