Employee Accidently Drinks Methanol and Is Poisoned

On June 28, 2006, methanol was poured from a one-gallon container into an empty water bottle. The water bottle was not relabeled to identify it was being used for methanol. The bottle was placed in the vicinity of Employee #1's water bottles which were collected and taken home at the end of the day. The following morning, June 29, 2006, Employee #1 drank from the bottle containing methanol and experienced immediate effects including vomiting, vision distortion, and disorientation. Employee #1 was transported to the hospital and hospitalized for methanol poisoning.
