Investigation Summary
On December 17, 2005, Employee #1 was working for a company that provided sanitary services. He was using an 8 foot high mobile ladder stand that had been modified. Eight inch rubber tires and a platform in front of the top step had been installed to allow employees to step out onto the top of the tanker truck to collect oil samples. The modifications, however, created an unstable condition. The mobile ladder stand platform was already in the "down" position when Employee #1 pushed it against the truck's tank side. Employee #1 locked the wheels at the ladder base and climbed to the platform. When he walked out onto the platform toward the truck, the ladder tipped forward, and Employee #1 fell 8 feet to the pavement. He sustained an unspecified injury to his foot or ankle, and he was killed. The mobile ladder went across a paved drainage ditch but never fell over. There is a 12 foot mobile ladder onsite that is also used to collect oil samples. It was also modified at the employer's request.
