Employee Is Killed When Crushed by Machine

Employee #1 was operating a door machine, when he called for help. The supervisor arrived first and asked Employee #1 where he needed to go next, since Employee #1 was a relief operator. The supervisor told Employee #1 to go ahead, and he would explain the machine problem to maintenance personnel. The regular operator of the machine returned from break, and the supervisor told her to stay there, and he would move the machine so she would not have to board in front of an open coke oven door with very dangerous and extreme heat. The supervisor then moved the machine south approximately 4 feet and then back north 4 feet. The supervisor heard the radio and the maintenance electrician yelling, "Don't move, man down." Employee #1 died at the hospital from crushing injuries.
