Employee Receives Burn from Generator

On May 5, 2006, Employee #1 had been assigned to trace and relocate wiring from an old generator system that was going to be replaced. The foreman had assigned the work and had indicated that the worker was suppose to be tracing lines and remove/rewire jumpers from the old panel and reconnect them to the main panel. The employee was cleared to perform the work. He had opened the old panel and taken the cover off. He had also opened the main panel and taken the cover off that as he was trying to trace wiring. At this point, live parts at 120VAC were exposed and Employee #1 became burned. He was not wearing nonmelting/fire rated clothing, safety glasses, hard hat nor insulated gloves. He was not using insulated tools either. His shirt consisted of a company issued sweat shirt, which according to the label, was a 50/50 cotton poly blend. According to the foreman, none of that equipment was used at voltages of less than 480/277 VAC. He was hospitalized with burn to hand and arm.
