Employee Is Killed, Another Injured When Trench Collapses

On February 09, 2007, Employees #1 and #2 were working by hand with shovels leveling the ground for the placement of a 24 inch concrete RCP storm drain that had previously been installed by the company and then removed for relocation 12 inches north. The employees were working directly near or next to an unprotected 7 foot vertical dirt wall containing a 10 inch concrete slab that ran the length of the wall from the remnants of a previous parking lot that had been removed. The vertical wall where the employees were working was not shored, benched, or sloped. While working near the relocation of a vertical wall, a portion of the embankment collapsed trapping Employee #2 by the legs and covering Employee #1. A large portion of the concrete slab also trapped Employee #1 and killing him. Employee #2 received a sprained left ankle when he was trapped by falling dirt and concrete from a 7 foot vertical dirt wall embankment where he was working. Employee #2 was hospitalized and treated for sprained ankle.
