Investigation Summary
On February 1, 2007, Employee #1, the first-shift lead mechanic/assistant foreman, was working in a tractor trailer maintenance shop. After he helped guide a coworker who was backing a trailer into Bay 16, he returned to working on a trailer that was parked in Bay 9. When the coworker attempted to uncouple the tractor from the trailer, the trailer rolled backwards and crushed Employee #1.
