Employee Is Struck by Terminal Tractor and Is Killed

At approximately 1:50 a.m. on August 13, 2006, Employee #1, a truck driver, was struck by a single axle terminal tractor as it was moving a loaded big rig trailer from a shipping dock to the trailer staging area. An employee of the terminal facility was driving a Magnum terminal tractor and began to execute a u-turn in the plant access road way where Employee #1 was walking. The lighting at the plant was located on the right side of the road way causing the terminal tractor and the trailer to cast a shadow on the left side of the tractor and trailer as they moved down the road way. Employee #1 was wearing dark colored clothing while he was walking in the middle of the road way. This placed Employee #1 on the left side of the tractor trailer as it approached him. The operator of the terminal tractor did not see Employee #1 walking in the roadway until he began to execute the u-turn and struck Employee #1. The impact caused Employee #1 to fall backward striking his head on the asphalt road way. Employee #1 was killed.
