Employee Is Struck and Killed by Falling Debris

On August 14, 2006, a group of employees were working on a demolition and remodeling project. A Bobcat skid steer loader was being operated on the 20th floor of the building to clean up concrete, brick, steel and other demolition debris. The operator, while moving the Bobcat forward to scoop up a load of debris, struck a piece of angle iron buried under the concrete debris pile, pushing it forward and striking a partially demolished brick wall approximately 8 in. in height, causing the interior and exterior bricks along with three large decorative concrete blocks to dislodge and tumble over the side of the building. Employee #1, an employee of the demolition firm, was struck by falling concrete debris while working at the ground level next to a tractor-trailer combination that he had just guided into position for the loading of demolition debris. Employee #1 was killed. Employee #2, an employee of a trucking company, was also struck by falling concrete debris that penetrated the roof of the tractor-trailer he was operating. Employee #2 was also killed, but the original report did not list this fatality. (See 200783934 for information on the other fatality.)
