Employee Is Killed When Crushed by Overturned Forklift Truck

On April 15, 2005, Employee #1 was riding on the right side of a forklift truck, while the operator was maneuvering the vehicle backwards down an inclined dock ramp. The operator sharply turned the steering wheel to the left, causing the forklift truck to overturn onto its right side. Employee #1 was crushed and killed.
