Investigation Summary
On October 14, 2005, Employee #1, a foreman, was working on the roof of the building under construction with another employee. There was an unguarded opening in the roof, 16 ft by 16 ft by 18 ft above a concrete floor. The foreman stepped into the opening and fell to the floor below. He was killed.
