Investigation Summary

Investigation Nr: 200631620
Event: 07/24/2007
Four Employees Sustain Second Degree Burns in Flash Fire

On July 24, 2007, Employee's #1, #2, #3, and #4 were working in the kitchen area of propane stove and grill test site. The burners on the stove had been lit, but did not appear to be getting enough gas to enable the flame to be adequately turned up for cooking. The burners were turned off and one of the employees attempted to light the stove with fluid-type lighter. Apparently there was a propane leakage and flash fire started. The four employees sustained second degree burns and were transported by helicopter to the Washington D.C. Burn Center. Employee's #1, #2, #3, and #4 were hospitalized. The incident appears to have been caused by an ignition of gas in the room. The Fire Marshal's report indicated that there was an unused gas line that was uncapped, and he also noted burn marks on the wall behind the line. His assessment was that the ball valve could have inadvertently been bumped causing gas to leak out of the uncapped line.
