Investigation Summary
On November 25, 2006, Employee #1 was placing archived file folders into a five drawer file cabinet. She was working alone in a small garage near the employer's place of business. She filled the drawers starting with the top drawer, which made the cabinet top heavy. The cabinet was not secured to the floor, wall, or other cabinets. Employee #1 had filled the top two drawers and half of the middle drawer. She was pinned against a vehicle parked in the garage when the cabinet tipped over. She was found dead, in a partially standing position, pinned between the cabinet drawers and the vehicle.
