Investigation Summary
At approximately 3:00 p.m. on April 10, 2007, Employee #1 was working at a facility that is an assembly and packing plant of premade snack product that the company packages into larger quantities. At the time of the accident, witnesses state the operator of the forklift (Caterpillar 5000-pound capacity forklift, Model Number GC25K, Serial Number AT82C-02944) was backing up and Employee #1 had just returned to his work area which was approximately 20 ft directly behind the forklift.. The operator of the forklift struck Employee #1, and had started to pull forward when Employee #1 yelled at the operator. The forklifts momentum had knocked down Employee #1 and his pants had gotten caught between the counter weight and the rear (steering) wheels of the forklift. The operator did not see anyone so he began to pull forward at which time Employee #1 and a supervisor began to yell. The operator placed the forklift in reverse again and ran over Employee #1's leg. Employee #1 received a fractured ankle and amputation of all five toes on his right foot. Employee #1 was taken to the hospital where his toes were reattached. Employee #1 was hospitalized.
