Employee Is Lacerated by Shattered Glass and Later Dies

At approximately 3:40 p.m. on March 23, 2007, Employee #1 and a coworker were assigned to inspect and silk screen a number of annealed glass sheets that farm equipment windshields are made from. The task requires the employees to lift the glass sheet onto an inspection table and then onto a different table for silk screening. The glass sheet is approximately 62-in. by 75-in. and weighing 95 pounds. During the inspection the employees discovered a defect, had lifted the sheet of glass off the table and were carrying it to a reject area. The coworker stated that her fingers were slippery and allowed the glass to contact the floor causing it to shatter. Employee #1 was struck in the neck by a piece of the shattering glass causing a laceration of the external jugular vein and larynx. Personal protective equipment was provided to protect the arms from glass lacerations but no other protective equipment was worn to protect other parts of the body. Employee #1 was transported to the nearest hospital and later flown to a larger trauma center where she later died. The official cause of death is listed as exsanguination (loss of blood).
