Investigation Summary
At approximately 5:00 a.m. on March 11, 2007, Employee #1 was cleaning the magnet and screen on the AB line in the beverage department, which is a routine task performed at least once per shift. The company's procedure called for the isolation of two butterfly valves in the sterilization unit to block the area of the piping that contained the magnet and screen, and the opening of a drain valve to relieve the liquid under pressure in the system. After starting the valve isolation process, Employee #1 was called away to perform another task. When Employee #1 returned, he attempted to remove the clamp on the screen, for removal and cleaning. The employer's written magnet and screen cleaning procedure did not require the employee to utilize lockout-tag out during the procedure. Employee #1 had not completely isolated the system, nor confirmed that the piping was under zero pressure. The piping contained hot water (approximately 210 degrees Fahrenheit) used to sterilize the system. When Employee #1 loosened the clamp on the screen filter, the hot water, under pressure was released from the system. Employee #1 sustained second-degree thermal burns on his chest, abdomen and thighs and was hospitalized.
