Investigation Summary
On December 29, 2006, an employee was helping a fork truck operator to spread the forks on a Caterpillar fork truck. The forks came off the mask, catching the employee's hands between the fork and the concrete ground. The employee' fingers were smashed. Three fingers from the right hand and one from the left hand were surgically removed, but the employee was not hospitalized. The forks were extra long and approximately six foot in length and weighted an approximate 150 lbs. The work environment was inside and had no contributing weather conditions.
