Investigation Summary
On September 11, 2006, Employee #1 was operating a bottle labeling machine, IML PPK labeler. The labels began to fall from the label basket during loading, while the machine was still running. Employee #1 reached up to grab the falling labels when her hand was struck by the labeler head, which was transferring the labels from the label basket to the bottles being labeled. The labeler head was approximately 4 in. by 5 in., and a full stack of labels in the label basket was approximately 16 in. Employee #1 lacerated her left middle finger.
