Employee Is Killed When Crushed Under Forklift

On September 29, 2005, Employee # 1 was riding on the right side of a 1979 Hyster forklift Model Number H360XL. The forklift operator made a left turn, which caused Employee #1 to slip off of and fall under the right rear wheel. Employee #1 was crushed and killed.
