Investigation Summary
On November 12, 2005, Employee #1 was installing metal roof sheathing on a 5-in-12 pitched roof. The metal roof sheathing was wet and slippery due to rainfall. Employee #1 was not wearing fall protection. He slipped and fell, about 13 ft, striking a dumpster and then the ground. Employee #1 suffered a closed head injury and died three days later.
