Employee Is Killed When Struck by Motor Vehicle

On February 14, 2006, Employee #1 was installing security chains on drain grates, as well as patching cement walls of the drains. After completing the work on two drains and he was working on a third drain, a truck struck and killed Employee #1. The accident investigation revealed that for the work on the first two drains, Employee #1 had parked his truck close to the drain and then placed a traffic cone outside his truck when he was working. However, when working on the third drain, Employee #1 only placed a traffic cone nearby. Additionally, Employee #1 was not wearing a safety vest, which the employer was cited for. During the interview of the truck driver, he reported that he saw a traffic cone near where the accident occurred, but did but did not see anyone working.
