Employee Is Struck and Killed by Falling Forklift Truck Mast

On February 1, 2006, Employee #1 was working as an industrial truck mechanic. He was removing a mast from a forklift. The mast was attached with metal chains, in the vertical position, to another forklift. The mast fell from its position, pinning and crushing Employee #1. He was killed.
